I'll take a stab at adding another category:
The secret state 4GW - Cosa Nostra (Sicilian version, not US).
Heh. I had to research that term. An interesting addition and one which I might well apply to the US mafia during the early 20th century. Or even mid-20th century if the mafia/JFK assassination conspiracy carries weight.
